Innovative strategies are vital to enhance patient engagement, care delivery and quality improvement as healthcare evolves. This session will explore a unique model that extends primary care beyond in-person visits to improve value-based care outcomes for high-risk older adults. Vytalize Health’s innovative "Priority Care Program" identifies high-cost Medicare patients and provides extensive clinical and social services, addressing appointment gaps and ensuring patients have access to needed care between in-office doctor’s visits. These coordinated services include in-home nursing, telemedicine, remote monitoring and other support aimed at curbing unnecessary ER visits and enhancing continuity of care. Results demonstrate the highly personalized approach is enhancing outcomes while curbing costs. The most recent data shows that Priority Care resulted in a 16% reduction in inpatient spending, 27% reduction in emergency room visits, and 68% reduction in total outpatient spending.
